If you require information about yourself or your medical records, please use the&nbsp;<a href=\"https:\/\/www.nhslothian.scot\/yourrights\/data-protection-privacy\/requests-for-personal-information-including-medical-records-subject-access-requests\/\">Data Protection Act Form<\/a>.<\/p>\n\n\n\n<p>The Freedom of Information (FoI) Act came into effect on 1st January 2005, ushering in a new era for the public sector in Scotland and making public services more transparent than ever before.<\/p>\n\n\n\n<p>The FoI Act gives people new rights to access recorded information held by public authorities.<\/p>\n\n\n\n<p>NHS Lothian policy is to publish responses to requests for information under the FoI Act.
